Heart&Stroke Hypertension Collaborative 2010 Keynote Presentation: The Next Frontier of Chronic Disease Prevention and Managemen
Keynote speaker: Dr. Michael Hillmer, PhD, Ontario Ministry of Health and Long-Term Care.
In his keynote presentation at the Heart&Stroke Hypertension Collaborative on March 27, 2010, Dr. Michael Hillmer PhD, Manager of the Chronic Disease Unit in the Health System Policy and Relations Branch of the Ministry of Health and Long Term Care of the Ontario Government, reviewed the Ontario Chronic Disease Prevention and Management (OCDPM) framework implemented in Ontario with the goal of improving clinical, functional and population health outcomes.
He discussed the impact of increasing multiple risk factors in the population which were contributing to the increasing prevalence of many chronic diseases including diabetes and hypertension.
Dr. Hillmer further talked about the need for widespread behavioural change and the opportunities that existed for further progress using the initiatives within the framework, and also discussed the challenges and practicalities of implementing these important initiatives.
