Taking Control of Type 2
The Taking Control of Type 2 diabetes program (TCT2) is an innovative and evidence-based, yearlong,
28-session program that empowers participants to embrace a balanced approach to diabetes self-care. TCT2 uses three pillars of support to encourage self-management of type 2 diabetes: medical management, healthy eating, and physical activity. Sessions are weekly for 16 weeks, twice a month for 4 months, and once a month for the last 4 months of the program. Each session is 1 hour long.
Guidance, support, and education are provided by registered nurses, certified diabetes educators, registered dietitians, pharmacists, and physical therapists. That instruction is complemented by a YMCA curriculum that covers motivation, goal setting, balanced eating, and achieving a healthy weight. Each participant is also paired with a YMCA Lifestyle coach, who helps each individual throughout the program. Through group discussion, hands-on learning, and practical application of knowledge, participants are given the tools they need to start and maintain a more healthy lifestyle.
