Improving Chronic Illness Care was a national program of The Robert Wood Johnson Foundation that worked for more than a decade with national partners toward the goal of bettering the health of chronically ill patients by helping health systems, especially those that serve low-income populations, improve their care through implementation of the Chronic Care Model. The Improving Chronic Illness Care program ended in 2011, but its core team remains at the MacColl Center for Health Care Innovation.
